Please complete the information below to register with UOW Student Accessibility and Inclusion service. The Student Accessibility and Inclusion team assists students experiencing temporary or permanent disability or medical condition/s to harness their strengths and overcome institutional barriers in order to fully participate in their study. The team of allied health professionals and specialised professional staff, works in partnership with students to enable them to achieve their study and career goals.

The more information you provide will assist us to effectively engage with you and assess your needs.

Information about the nature of your disability or medical condition will be handled in a confidential manner and will not appear on your academic records.

When you submit this registration form, you are acknowledging the following:

I acknowledge and agree that the Student Accessibility and Inclusion team shall arrange assistance and support.

  • All relevant student details and subject information held in University’s student systems, may be accessed by the SAI Team in order to appropriately communicate your needs.
  • My Access Plan may be shared within the Student Accessibility and Inclusion Team and relevant university staff who need to know specific information to implement any adjustments (such as Academic teaching staff, Exams team).
  • Relevant information may be shared with a dedicated support person, if one is required.
